What is a gummy smile, and what causes it?

A gummy smile is when a noticeable amount of gum shows above your teeth as you smile, often with the top lip appearing quite slim. There's no fixed medical definition — it comes down to how balanced your smile feels to you.

It's usually caused by the upper lip muscles lifting the lip higher than average when you smile. A small dose of botulinum toxin is placed above the cupid's bow to relax those muscles, so the top lip doesn't rise as far — showing less gum and giving the lip a slightly fuller, softer look.

Aftercare

Looking after your results

  • Stay upright for around 4 hours after your treatment.
  • Avoid touching, rubbing or massaging the area for 24 hours.
  • Skip strenuous exercise for 24 hours.
  • Avoid alcohol for 24 hours.
  • Avoid heat treatments — saunas, steam rooms and sunbeds — for about 2 weeks.
  • Hold off on makeup over the area for a few hours.
  • Results develop over 3–14 days, so be patient at first.

What to expect

After your treatment you may notice some stinging or slight bruising where the needle was placed. These effects are temporary and settle quickly.

Anti-wrinkle injections take several days to take full effect, so don't worry if you don't see a change in the first few days — this is normal.

How to prepare for your treatment
  • Avoid alcohol for 48 hours before your treatment.
  • Let us know about any prescribed or non-prescribed medication you're taking.
  • Avoid over-the-counter anti-inflammatories such as ibuprofen for 48 hours beforehand.
Who shouldn't have anti-wrinkle treatment

This treatment isn't suitable if any of the following apply. If unsure, please ask during your consultation:

  • Pregnancy or breastfeeding.
  • Hypertrophic or keloid scarring.
  • Bleeding abnormalities (e.g. anticoagulant use or thrombocytopenia).
  • Active skin condition in the area (e.g. eczema or psoriasis).
  • Active infection in the area (e.g. herpes simplex or pustular acne).
  • Impaired healing (e.g. due to immunosuppression).
  • Skin atrophy (e.g. chronic oral steroid use, or syndromes such as Ehlers-Danlos).
  • Sensitivity or allergy to botulinum toxin or its constituents, or a milk allergy with certain products.
  • Neuromuscular disorders (e.g. ALS, Lambert-Eaton syndrome, myasthenia gravis or myopathies).
  • Gross motor weakness in the area (e.g. Bell's palsy or stroke).
  • Uncontrolled systemic conditions.
  • Medications that may potentiate the effect (e.g. aminoglycosides, quinine, calcium-channel blockers, penicillamine).
  • Unrealistic expectations or body dysmorphic disorder.
  • Periocular or ocular surgery in the last 6 months.
